Raipur. In serious cases, health centers including district hospitals are making mistakes in referring patients to bigger medical institutions. Due to this, cases of death have started increasing. To reduce the number of deaths in referral cases, the Health Department has made a referral policy. Under this, the referring institutions will have to adopt the process of right time, right place, right situation, right process, right medium. In this regard, on behalf of the Directorate of Health Services and Director Epidemic Control, information about the guidelines has been given by sending letters to all the District Chief Medical and Health Officers, Civil Surgeons cum Hospital Superintendents of District Hospitals. They have been directed to follow these points while referring any patient to a higher institute.

According to sources, it is often seen that after seeing patients in serious condition in district hospitals or health centers, they are immediately referred to big hospitals. Many times this process gets delayed and due to not getting timely treatment, the patient loses his life. Keeping such incidents in mind, after the review of the Death Audit Committee formed at the state level, instructions have been issued to follow five correct points in the guidelines issued to reduce deaths. Along with this, instructions have been given to form a Death Audit Committee in every hospital and review such cases.

1. Right time – The medicines are not having the proper effect or the condition is not improving within 72 hours. Maroz needs special investigation. There should be no facility for any surgery and the patient needs it immediately. The family members themselves wish to go to another hospital and should be referred if the doctor gives consent for transfer.

2. For the right place- The patient should be referred to a medical institution where required facilities are available. For this, related information should be obtained first. Generally, there is a system to refer patients from primary health center to community health center, from community health center to district hospital and from district hospital to medical college.

3. In the right position – While sending the patient to another hospital for further treatment, it should be ensured that there are no complications en route. This procedure should be completed only after stabilizing the patient. If there is dehydration, fever, external bleeding etc., the patients should be transferred to another hospital only after first aid.

4. Sending through correct channel – An ambulance or capable vehicle will be used to send the patient to another hospital for treatment. If the patient requires oxygen or ventilator, he should be sent in a well-equipped ambulance.

5. Following the correct procedure – While referring the patient to another hospital, information related to him should also be sent to the hospital. Such as the condition during admission, treatment given, tests done, condition of the patient during referral should be mentioned.

Apart from this, hospitals have been directed that as far as possible, the medical staff should remain with the patient till he reaches another hospital. Prior information should be given to the higher hospital so that necessary preparations can be made. If possible, necessary information related to the patient should also be given to the concerned doctor over telephone.

Death audit should be done at hospital level

Instructions have been given by the Health Department to form a Death Audit Committee at the hospital level to identify the shortcomings and remove them in case of such deaths. The referral matter should be kept completely clear, so that no confusion is created between the staff of the higher institution and the family members of the patient.
